Detailed guides on how chronic disease management and Medicare care plans work in Australia

What A Care Plan
Actually Gives You

Medicare calls this a GP Chronic Condition Management Plan, or GPCCMP. It replaced the older GP Management Plan and Team Care Arrangement in July 2025, so one plan now does the job the two used to do. If you had a plan before then, your GP will move you across at your next review.

What Is In Your Plan

What To Bring To The Appointment

Allied Health
On Your Care Plan

A care plan gives you Medicare rebates for up to five allied health visits each calendar year, shared across the providers your GP refers you to. Your doctor will recommend which ones suit your conditions and can refer you to physiotherapists, podiatrists and dietitians on the northern Gold Coast, close to where you live. Some providers bulk bill and others charge a gap, so ask about fees when you book with them.

Dietitian Support

Eating plans for diabetes, cholesterol, kidney disease and weight, built around food you actually like.

Physiotherapy

Strength and mobility work for arthritis, back pain and recovery, so you keep moving with less pain.

Podiatry And Foot Care

Foot checks and ongoing care, especially important with diabetes or poor circulation.

Exercise Physiologist

Safe, structured exercise for heart disease, diabetes and chronic pain.

Psychologist

Support for the mental load of living with a long-term condition.

Diabetes Educator

Practical help with monitoring, medicines and day-to-day management.

Care Plan Questions,
Answered

What patients ask our Biggera Waters GPs about chronic disease management.

Generally yes if you hold a Medicare card and have a condition that has lasted, or is likely to last, at least six months. Diabetes, heart disease, high blood pressure, asthma, arthritis and kidney disease are common examples. Your GP will confirm at your appointment.

Up to five individual visits each calendar year, shared across all the providers on your plan. Medicare pays a rebate towards each visit. The number resets each January rather than 12 months from your plan date.

Medicare replaced the GP Management Plan and Team Care Arrangement with a single GP Chronic Condition Management Plan. Existing plans and referrals can still be used during the transition, and your GP will move you onto the new plan at your next review.

Your GP will set a schedule to suit your conditions, often every few months. Reviews keep your medicines, tests and allied health referrals current, and give you a chance to change your goals.

It helps a great deal. The doctor who knows your history will spot changes faster and keep your care joined up. You can request a particular GP when you book, and meet the team on our doctors page.

We are an accredited bulk billing practice, so appointments to prepare and review your plan are bulk billed for eligible Medicare card holders. Allied health providers set their own fees.

Yes. One plan covers all of your chronic conditions together, which is the point of it. Many patients have two or three conditions that affect each other, such as diabetes with high blood pressure.

MyMedicare is a voluntary registration linking you to one practice. If you are registered with a practice, your plan needs to be prepared there. If you are not registered, your usual GP can still prepare it. Ask reception if you would like to register with us.
